    a) Cơ sở pháp lý thành lập tổ chuyên gia:
    Tổ chuyên gia được [Ghi tên Chủ đầu tư/đơn vị tư vấn đấu thầu] thành lập theo Quyết định số [Ghi số và ngày ban hành văn bản] để thực hiện lập HSMT, E-HSMT gói thầu [ghi tên gói thầu] thuộc dự án/dự toán mua sắm [ghi tên dự án, dự toán mua sắm].
    Trường hợp Chủ đầu tư thuê đơn vị tư vấn đấu thầu lập HSMT, E-HSMT thì bổ sung nội dung sau:
    Căn cứ hợp đồng số [Ghi số hiệu hợp đồng] ngày [Ghi thời gian ký hợp đồng] giữa [Ghi tên Chủ đầu tư] và____ [Ghi tên đơn vị tư vấn đấu thầu] về việc thuê lập HSMT, E-HSMT gói thầu [Ghi tên gói thầu] thuộc dự án/dự toán mua sắm [Ghi tên gói thầu].
    b) Thành phần tổ chuyên gia:
    Số lượng, họ tên, chức vụ, vị trí và phân công công việc cụ thể của các thành viên trong tổ chuyên gia được nêu tại Bảng số 01.

    c) Cách thức làm việc của tổ chuyên gia:
    Phần này nêu cách thức làm việc của tổ chuyên gia, phân công công việc, thời gian hoàn thành... Trường hợp có quy chế làm việc của tổ chuyên gia thì chỉ cần đính kèm quy chế này. Trong phần này nêu rõ cách xử lý khi một thành viên trong tổ chuyên gia có ý kiến khác biệt so với đa số các thành viên khác. Ý kiến bảo lưu của thành viên đó (nếu có) phải được nêu trong báo cáo này.
    II. NỘI DUNG HSMT, E-HSMT
    1.     Nội dung của HSMT, E-HSMT
    HSMT, E- HSMT được đính kèm báo cáo này. Trong phần này có thể nêu rõ những nội dung quan trọng của HSMT, E-HSMT và lý do đưa ra các quy định này.
    2. Ý kiến bảo lưu của thành viên tổ chuyên gia
    Trường hợp có ý kiến bảo lưu thì cần nêu rõ các thông tin: nội dung HSMT, E-HSMT, ý kiến bảo lưu, lý do và đề nghị thành viên có ý kiến bảo lưu ký tên theo bảng dưới đây:

    III. KẾT LUẬN VÀ KIẾN NGHỊ
    Tổ chuyên gia nêu rõ các nội dung đã thống nhất và kiến nghị bên mời thầu trình chủ đầu tư xem xét, phê duyệt HSMT, E-HSMT, trong đó nêu rõ các nội dung cần lưu ý trong quá trình xem xét, phê duyệt HSMT, E-HSMT.
